Physicians Health Appraisal Form – Take this form with you when you go to the doctor for a sports physical. After complete, return to the school nurse.
Student Participation Form – Student and parent sign this form to acknowledge risks, rules and consent to out-of-town trips. This form goes to the coach and gives the coach contact information for the parent and student; also permission to seek medical attention, if necessary.
Athletic Health History/Interval Form – This form asks for a more extensive health history in addition to the Physicians Health Appraisal Form. Parent fills out and signs this form and gives it to the school nurse.
